Birth & Death Certificate Information

Requests for birth and death certificates can be made in person or by mail.

Requests made in person - If you come to Village Hall, the request will be processed while you wait - based on availability. Office hours for Birth and Death Certificates is  Monday - Thursday  9am - 3pm.
The cost is $10 cash or credit card per certified copy. You will need to present identification: 

What identification is acceptable by the applicant when applying in-person? 

Application must be submitted with the following proof of identification from Category A or B.

  1. One of the following forms of valid photo-ID:
    • Driver's license
    • State-issued, nondriver photo-ID card
    • Passport
    • U.S. Military-issued, photo-ID

OR

  1. Two of the following showing the applicant's name and address:
    • Utility or telephone bill
    • Letter from a government agency dated within the last six months

Below you will find information on how to request a copy of a Birth or Death Certificate by mail:

Who is eligible to get a copy of a Birth Certificate?
  • The person named on the Birth Certificate.
  • A parent of the person named on the Birth Certificate. The requesting parent's name must be on the Birth Certificate.
  • A spouse, child or others, but only by order of a New York State Court.

Birth Certificate Request - to request a certified copy of a Birth Certificate please include the following information in a letter:

- Birth name
- Date of birth
- Mother's maiden name
- Father's name  
- Sign and have the letter notarized.  
- Include a money order payable to the "Village of Nyack" in the amount of $10 per certified copy - no personal checks accepted.
- Please include a self-addressed stamped envelope so we can mail it back to you.

Death Certificate Request - to request a certified copy of a Death Certificate please include the following in a letter:

- Name of Deceased
- Date of Death
- Maiden name of Mother of Deceased
- Name of Father of Deceased
- Place of Death
- State your relationship to the Deceased
 - Sign and have the letter notarized

You must be a qualified applicant or relative in order to obtain a Death Certificate.  Qualified Applicants for Death Certificate include: spouse, domestic partner, child, sibling, or parent of deceased.
